PAR-Q Form

We require all trainees to complete a PAR-Q screening to ensure the instructor training is safe and suitable for you. This quick form helps identify any health considerations we should be aware of before you begin, so we can support you properly throughout the programme. Your safety and wellbeing are our top priority.

Complete beginner
Beginner - I’ve tried it a few times
Improver - I can hoop but still learning
Intermediate - comfortable with basic moves
Advanced - confident with a variety of moves

Can you hula hoop in both directions?

Are you a certified fitness instructor? For example: Level 3 Personal Trainer, Exercise to Music, Group Fitness, or Hooping Instructor Qualifications.
